Before you visit
Please ensure you have all required documents ready before you visit. In particular:
- Bring current ID (preferably photographic)
- Fill out but do not sign forms in advance
- Bring the original document(s) plus any copies you want certified
- We can certify printouts of some digital documents, but we will need to see the original on your phone/laptop including where it came from (e.g. an email or official website)
- We cannot accept scans or photographs of physical documents as originals
- If an interpreter is required, please organise one before attending

How to find us
We're located at the QEII Jubilee Hospital, Cnr Kessels Rd and Troughton Rd, Coopers Plains. The hospital's website has information about how to get there by car and public transport.
Once you've arrived, enter the hospital through the Main Entrance and head left at the lifts, then right as you pass the pharmacy. You'll see us at a desk to your left.
Parking
Parking is available in P2 (129 Middle Street), Coopers Plains. Look for the signs at the car park entrance. To get directions and pin for the P2 car park, see QEII Hospital Car Park on Google Maps.
If you need a wheelchair or need help getting to and from your car you can call the hospital's Transport Service 0477 821 250 for assistance.
